미국에서 레지던트를 하면서 임신 및 출산, 육아가 가능한 여건인가요? 

출산 휴가나 이러한 제도가 잘 마련되어 있는지 궁금합니다.

Not very good. I personally did not have a childbirth during residency, but from what I could see from colleagues, there are a few things to know.

There is no paid maternity leave guaranteed by law. All workplaces are required to let employees have parental leave up to 12 weeks (3 months) without firing them, but they are not required to pay them. So a lot of places will give employees short-term disability pay for 6 weeks, but the other 6 weeks will be unpaid. Now each individual employers can have better policies for their employees. For example, for my first child, my wife was attending physician and I was a fellow. My wife got 12 weeks maternity leave and was paid 75% of base pay for 6 weeks with the other 6 weeks unpaid. I was given 1 week paid leave for paternity leave. For my second child, my wife had stopped working and I was attending physician. We were in the same employer but their policy changed, so I received 8 weeks paid parental leave. So it is quite workplace-dependent, but probably not as good as in Korea.

For residents, there may be better support system, but you may also have to complete your residency later. Since US residencies do not run on strict schedules like in Korea, what will happen is you will graduate a few months later than other residents. This is what happened with one of our co-residents in residency. She had 2 months off for maternity leave, probably paid, then she graduated 2 months after everyone else in her year, but she was not required to repeat a year.

Childcare is really hard, and many people use their families, especially their parents, for childcare. Usually daycare centers are expensive, but nannies are even more expensive, but you will end up having to utilize these services if you don't have family around or if you don't have a stay-at-home spouse. It is possible, and many residents in the US have kids during residencies or even during medical school and somehow manage, but it is going to be stressful. The social support system is not as comprehensive in the US.
